If B accepts A’s foul then replay the down. Extend the game by an un-timed down (3-3-3b, 10-2-1c).  If B declines A’s foul, enforce B’s BIB at the spot of the foul. 1st and 10 for B at the 50, extend the game by an untimed down. A does NOT have the option to decline B’s foul (3-3-3a, 10-2-2).

Ruling: 10/02/2006
a.  Name the fouls on this play and its aftermath, if any.
There are two fouls on this play.  The first foul is an illegal forward handoff by A43 at B's 12 yard line.  This penalty includes a loss of down provision.  Therefore, there is no extension of the period and an untimed down.  The second foul is a dead ball unsportsmanlike penalty on B64 for slugging A68.
b.  Will the game go into overtime?  Why or why not?
Yes, the game will go into overtime.  The 4th quarter cannot be extended with a penalty that includes a loss of down, or a dead ball foul.
c.  If the game goes into overtime and A wins the coin toss, what is the down and
distance choices would you give the captain before he chooses offense or defense?
To start the overtime period, the dead ball foul on B64 will be enforced and he will be ejected. If A chooses to be on offense, they will be give the choice of 1st down from the 5 yard line (dead ball foul is enforced as half the distance).  If A chooses to be on defense, B will start 1st down from the 25 yard line after a 15 yard enforcement of the dead ball foul.
